If your child requires medication (prescription or over-the-counter) at school, please adhere to the following guidelines:
A healthcare provider’s (MD, DO, CRNP, or PA) note must accompany the medication. It should include:
your child’s name
date
name and dosage of medication
time for medication administration
A completed Medication Authorization Form (can be found here or is available from the school nurse)
A parent note requesting that medication is administered at school, as prescribed by your health care provider.
Prescription medication must be in the original prescription bottle with the prescribing doctor’s signature, the student’s name, and instructions for the administration of medication.
Over-the-counter medications must be in the original labeled bottle.
